Insurance Claims

We want to make every aspect of treatment easy for you, especially when it comes to handling insurance claims. Below are a few tips to make sure everything is processed smoothly so that you can focus on achieving the healthiest you:

  • Check your insurance provider manual to be sure your plan allows you to be seen by one of our providers.
  • Many plans require a referral to be seen by a specialist, so please be sure to obtain one prior to your visit.
  • Carefully check your “New Patient Insurance Form” for accuracy after completing.
  • Notify our office of any changes with your insurance prior to your next appointment.

We participate with Medicare, many commercial insurances, and managed care health plans. Click the list of approved insurances below to see the participating health plans we accept.

We do our best to keep up to date with our list of participating plans; however, we encourage you to call us if you don’t see your plan on the list because the list is subject to change periodically. (702) 724-8877.

Payment Information

Some insurance plans require that you pay a co-payment and/or deductible for your visit. You are expected to resolve any outstanding balance not covered by your insurance company. Here is a list of different forms of payment we accept:

  • Cash
  • Personal checks
  • Visa
  • Mastercard

We do not hold checks or accept post-dated checks. Additionally, we ask for advance payment of co-payments, co-insurance and/or unmet deductibles prior to scheduling surgery. Please call our billing department if you have any questions or concerns regarding billing issues. (702) 724-8877.
